• People recently discharged from a hospital or rehab unit.
• People who live in the Denver metro area, Canon City, Trinidad and Pueblo.

Total Longterm Care (TLC) provides coordinated healthcare, home care and support to help aging individuals live independently in their community.

Who is it for?

• People who are 55 years old or older and live in one of TLC's service areas in the Denver metro area or Pueblo.
• People who are certified by the state of Colorado as meeting the need for nursing home level of care.
• People who are able to live safely in the community with the help of TLC.
